Applying to Live at Foyer Oxford

Application Form - To be completed after the registration form

This is for young people to complete & submit to Foyer. Please ensure you complete Essential Criteria check list below before you start filling out the application form. In order to submit your application, you will be asked to consent to the storing of your information. If you are concerned about the privacy of your information, please scroll to the bottom of the form before completing your application. 

Essential Criteria

You MUST be able to tick all of the boxes otherwise you are not eligible to live at Foyer Oxford.

Information Session Attendance

Foyer will NOT be able to process your application until we can verify that you have attended an information session.

Foyer will not accept this application if you have not been to an info session.

You need to be working or studying to live at Foyer Oxford. At your assessment interview you may/ will be asked to provide evidence such as enrollment forms, attendance records and/or payslips

  • Everyone who lives at Foyer Oxford program needs to be able to commit to full-time study, work or training.
  • If you are not working or studying, and it has been more than 6 months since the last time you were in work or study, Foyer Oxford may not be able to offer you accommodation yet.
  • If you have just started working or studying after a long break, Foyer Oxford may ask you to show that you can stay in work or study for at least 10 weeks before we are able to offer you accommodation.

Sharing of Information

Usually, everything that you fill out in this form is confidential. This means the information stays with the Foyer Oxford team and we won’t share it without your permission. If we believe that you or others are unsafe we would need to break this confidentiality. If we think this is the case then we will speak to you before we do anything.

  1. I give permission for Foyer staff to contact me about this application. I also give consent for my referees and the person who helped me fill this out the form to share information relevant to this application.
  2. I understand that Foyer is comprised of Anglicare WA, Foundation Housing and North Metropolitan TAFE. I give permission for these agencies to share information including information from any previous contact I have had with these agencies.
  3. The Specialist Homelessness Services (SHS) Collection needs information to help people who experience homelessness or are at risk of homelessness. Your information will be passed on to the Australian Institute of Health and Welfare, but will not contain any information that identifies you, nor will your information be passed on to any other organisation. Click here to read their Privacy Information Card. 

SPECIALIST HOMELESSNESS SERVICES COLLECTION

The Specialist Homelessness Services (SHS) Collection needs information to help people who experience homelessness or are at risk of homelessness. Your information will be passed on to the Australian Institute of Health and Welfare, but will not contain any information that identifies you, nor will your information be passed on to any other organisation. Click here for more information. 

Previous Tenancy Check

I give consent to Foundation Housing Ltd to check their database for any previous tenancies and/or debts I may have with them. Foundation Housing has the right to reject your application if you have previously caused malicious damage to one of their property or you still owe them a large amount of money.

Please ask to talk to a Foyer worker if this affects you.

Consent Period: The consent to share information between the consortium agencies will remain valid until your time in the Foyer Oxford program is completed or unless it is withdrawn by you.
